Next Practice Deakin is delivering a Healthy Ageing Program for eligible older people living in the ACT community who may be pre-frail, living with frailty, or noticing changes in strength, energy, weight, mobility, confidence or wellbeing. The program is designed to identify needs early and connect people with coordinated, person-centred care. Referrals and self-referrals are accepted through the online referral form. Participation is voluntary and based on eligibility, consent and clinical suitability. This Healthy Ageing Grant activity is supported by funding from the ACT PHN through the Australian Government’s PHN Program.
Next Practice Deakin is delivering aHealthy Ageing Program to support eligible older people living in the ACT community who may be pre-frail, living with frailty, or noticing changes in strength, energy, weight, mobility, confidence, wellbeing or daily function.
The program is designed to help identify needs early and connect people with coordinated, person-centred care. It focuses on areas that can affect healthy ageing, including movement, strength, nutrition, medication safety, wellbeing, social connection and access to appropriate supports.
Participation is voluntary and based on eligibility, consent and clinical suitability.
The program is for people living in the ACT community who are aged 65 years and over, or Aboriginal and Torres StraitIslander people aged 50 years and over, and assessed as pre-frail or frail using the Morley FRAIL Scale. Self-referrals are accepted.
The program is not designed for people living in residential aged care homes.
Depending on individual needs, the program may include physical activity, strength, mobility and balance; nutrition and healthy eating support; medication safety and medication burden support; psychosocial wellbeing, confidence and social connection; care coordination and connection with community services; support involving families or carers, where the person gives consent; and assessment/review of health, function and wellbeing measures over time.
The program works alongside usual GP care and other existing health and community supports.
As part of the program, we may ask participants questions about their health, wellbeing, daily function, care experience and practical support needs.
These questions may cover strength, energy, mobility and daily activities; mood, pain, fatigue, sleep and social connection; nutrition, appetite and weight; medication burden and medication safety; falls, emergency department visits or hospital use; confidence in managing health; experience of care; transport, housing, food access, safety, language, digital access and other practical things that can affect health.
These are sometimes called patient-reported measures, experience measures, functional measures and social needs questions.We ask these questions to help the team understand what support may be useful, coordinate care with consent, and evaluate and improve the program. Information is handled in accordance with privacy requirements. Program reporting uses de-identified information where required.

The Morley FRAIL Scale is required for eligibility. If it has not already been completed, people can complete the Healthy Ageing check online by clicking https://deakin.nextpracticehealth.com/forms/healthy-ageing-check-strength-energy-and-weight-questions
Once a referral is received, a dedicated Registered Nurse will review the referral and contact the participant or nominated contact within 3 working days.
This program does not replace usual GP care, emergency care, hospital care, crisis support or specialist medical care.
