Welcome to Next Practice Deakin Disability Services. Our NDIS Support Coordinators partner with you to get the most out of your care plans and practitioners by offering Level 2 Support and Level 3 Specialist Support services.

Level 2 and Level 3 Coordination Services

At Next Practice Deakin Disability Services, we offer both Level 2 Support and Level 3 Specialist Support coordination for patients with a NDIS care plan.

Level 2 and Level 3 supports are progressive tiers of assistance designed to match the complexity of a participant’s needs. Typically, Level 2 Support offers hands-on, practical help with daily coordination tasks: setting up services, linking you with community activities, supporting you to attend appointments, and helping you use funded supports effectively.

Level 3 Specialist Support is more intensive and specialised, which can be best for people with complex or high-risk needs; it includes specialist coordination, crisis planning, advocacy with multiple agencies, detailed implementation of behaviour or health-related supports, and ongoing monitoring and review.

To ensure you're getting the right level of assistance, we tailor the level of support to your situation by combining regular check-ins, meetings, or joint advocacy as needed while aiming to increase your independence where possible.
