Identifying and Supporting People with Long COVID

At Next Practice Deakin, we remain committed to the ongoing health and well-being of our community, especially those who may still be feeling the effects of COVID-19 weeks or months after their initial infection.

Some people recover quickly, but others may experience ongoing symptoms such as fatigue, breathlessness, brain fog, pain, anxiety, or difficulty managing everyday activities. These symptoms are commonly referred to as Long COVID or Post-COVID Condition.

We’re pleased to announce that we’ve received a small amount of funding from Capital Health Network to support a focused initiative aimed at identifying and supporting patients affected by Long COVID.

As part of this project, we are inviting anyone who may be experiencing ongoing symptoms to complete the Yorkshire COVID-19 Self-Assessment Tool. This is a validated, secure, and confidential survey designed to help identify post-COVID impacts across a wide range of physical, emotional, and functional health areas.

📝 Take the survey here: https://bit.ly/NPDC19-YRSm
(The survey takes approximately 10–15 minutes to complete)

Once completed, your responses will be reviewed by one of our experienced nursing team members. Based on your results, you may be invited for a follow-up discussion where we will work with you to develop a personalised management plan. This may include:

  • Support from our multidisciplinary care team
  • Access to services such as physiotherapy, dietetics, psychology or speech pathology
  • Referral to relevant specialist services if appropriate

All information provided through the survey is treated with the highest level of confidentiality and data security, and is used solely to support your recovery and health needs.

If you or someone you care about is living with ongoing symptoms after a COVID-19 infection, we encourage you to take this important step. Early identification and tailored support can significantly improve health outcomes and quality of life.

Please reach out to our team if you have any questions about the survey or the support available through this program.
