Opportunity for GP(s) interested in Aged Care

NPD is a contemporary and progressive general practice in the heart of Canberra’s medical precinct. As well as opportunities for in-practice general practice we’re seeking to collaborate with doctors interested in Aged Care and disability care.

Doctors from Next Practice Deakin currently provide in-reach services to retirement villages, aged care facilities, and disability homes across Canberra. The opportunity would suit a GP with an interest in managing people with complex and chronic conditions and/or palliative care.

  • As an experienced medical centre in providing these services GPs are supported by:
  • A multi-disciplinary team of patient advocates, registered nurses, nurse practitioners, practice pharmacist, and social worker
  • Cloud-based electronic health records and telephony systems are provided to support flexible working
  • A bespoke communication system with RACFs using a secure communication portal
  • Co-ordinated internal cover arrangement during leave as the team of clinicians grow
  • Telehealth options include enhanced Telehealth in some RACFs to remotely examine patients including looking in ears, auscultation of lungs and hearts and assessing skin lesions and wounds
  • Mentoring and support if you are new to aged care
  • Internal clinical meeting for professional development and peer support

Australian Menopausal Society Symptom Score Card

The AMS Symptom Score Card is a short questionnaire designed to help you and your doctor understand the severity and impact of your menopause or perimenopause symptoms. It allows you to record symptoms such as hot flushes, sleep disturbance, mood changes, cognitive changes, vaginal dryness and other physical or emotional concerns. Each symptom is rated on a simple scale, helping track changes over time. Completing the score card before your appointment helps your doctor: 1. Understand your most concerning symptoms 2. Assess how menopause is affecting your daily life 3. Guide decisions about treatment options 4. Monitor your progress over time We encourage you to complete the scorecard before your menopause consultation and bring it with you, or submit it via your online form if provided.
