Pre-Appointment Information for Your GP Visit with Dr. Paresh Dawda

  • Please arrive on time and allow a few minutes before your appointment to check in at reception. You may be asked to confirm or update your personal and contact information to ensure our records are accurate.
  • Bring any relevant health documents, including a list of current medications, recent test results, or referral letters if they have been done elsewhere.
  • Prepare a list of your main concerns or questions to help make the most of your time with the doctor.
  • During your visit, your GP may use a secure digital scribe to assist with clinical documentation. This allows your doctor to remain fully engaged in the conversation while ensuring your concerns are accurately recorded. If you have any concerns or questions about this, please let your doctor know.
  • We encourage you to consider the Choosing Wisely Australia "Five Questions" to support informed decision-making during your care:
    1. Do I really need this test, treatment or procedure?
    2. What are the risks?
    3. Are there simpler, safer options?
    4. What happens if I do nothing?
    5. What are the costs?

Australian Menopausal Society Symptom Score Card

The AMS Symptom Score Card is a short questionnaire designed to help you and your doctor understand the severity and impact of your menopause or perimenopause symptoms. It allows you to record symptoms such as hot flushes, sleep disturbance, mood changes, cognitive changes, vaginal dryness and other physical or emotional concerns. Each symptom is rated on a simple scale, helping track changes over time. Completing the score card before your appointment helps your doctor: 1. Understand your most concerning symptoms 2. Assess how menopause is affecting your daily life 3. Guide decisions about treatment options 4. Monitor your progress over time We encourage you to complete the scorecard before your menopause consultation and bring it with you, or submit it via your online form if provided.
