Outreach Service – Information for Residential Aged Care Homes

Our outreach service is designed to make it easier for Residential Aged Care Homes (RACHs), residents, and substitute decision-makers to connect with the Next Practice Deakin team. The links below bring together the key information, forms, and telehealth access details in one place.

For New Residents and Enduring Powers of Attorney (EPOAs)

Please use the following links when a new resident is being referred to our outreach service. These can be shared with residents, families, and EPOAs.

Ongoing Communications and Telehealth

For day-to-day communication with our team and to support telehealth consultations, please use the options below.

Online Communications Log (for RNs)

Registered Nurses can use the online communications log to share updates, requests, and clinical information with our outreach team.

Note for RNs: Please complete a separate entry for each resident you are contacting us about. Include relevant background, current concern, and any time-sensitive issues so the team can prioritise safely.

Telehealth – Healthdirect Video Call

For scheduled telehealth appointments with our clinicians, please use the Healthdirect Video Call link below. This can be used on a facility device or with resident and family present, as appropriate.

Please ensure the device has a working camera, microphone, and stable internet connection before the appointment time.
