Palliative Care

Palliative care at Next Practice Deakin focuses on supporting people living with life-limiting illness to maintain comfort, dignity and quality of life. Our doctors and nurses work in partnership with patients, families and carers to provide coordinated, person-centred care that aligns with individual values, goals and preferences.

Our palliative care services include:

  • Symptom assessment and management, including pain, nausea, breathlessness and other concerns
  • Advance care planning and support with documenting preferences
  • Medication review and deprescribing where appropriate
  • Emotional, psychosocial and practical support, including connecting patients and families to community services and allied health
  • Coordination with specialists, hospital teams, home-based nursing and aged-care providers
  • Home visits for patients who are house-bound or in residential aged care (where available)

We aim to provide care that is compassionate, respectful and responsive to changing needs. Whether delivered in the clinic, in the home or in collaboration with broader care teams, our approach supports patients and their families through every stage of their palliative care journey.
